Pharmacological action

The premature ejaculation is a common sexual function-related male disease that is always or almost always within a minute of vaginal insertion, and refers to cases in which the assessment is made, the assessment cannot be delayed, and the resulting stress and frustration lead to mental pain or influence such as avoiding sexual relationships. Based on research results that show that the concentration of serotonin, a neurotransmitter, may decrease in the area in charge of ejaculation of the brain, drugs that increase the concentration of serotonin are used as oral premature ejaculation.

Dafoxetin is a selective serotonin reuptake inhibitor (SSRI) family of drugs that treat premature ejaculation by increasing the concentration of serotonin in the central nervous system. The nerve cells in the brain are connected to each other through a structure called synapse, and the signal is transmitted by binding the neurotransmitter secreted from the end of the nerve cell to the receptor of the opposite nerve cell. The neurotransmitter secreted from the end of the nerve cell is lowered in concentration by reabsorption, and selective serotonin reuptake blockers such as polyfoxetin increase the serotonin concentration in the synapse by blocking the reuptake of serotonin, thereby increasing the action. Unlike other selective serotonin reuptake blockers generally used to treat depression, Dafoxetin is used as an oral premature ejaculation because it is quickly absorbed and discharged in the body.

Sexual function-related drugs on the market in Korea are designated and managed by the Ministry of Food and Drug Safety as drugs of concern for misuse in order to prevent them from being misused socially, unlike the originally approved purpose of treatment. Dafoxetine is also designated as a drug of concern for misuse due to the difficulty of distinguishing between erectile dysfunction and premature ejaculation, and must be used under the guidance of experts.

Efficacy/Effectiveness 

It is used to treat premature ejaculation in adult men (ages 18 to 64). There are products such as Prelidge.

how to take it

About 1 to 3 hours prior to sexual activity, 30-60 mg (first dose can be increased to 30 mg, if the effect is not sufficient, up to 60 mg) is administered regardless of the diet. Only one dose can be administered within 24 hours, and it should be swallowed at once to avoid bitterness and administered with at least 1 cup of water.

After the first 4 weeks of administration or 6 doses, it should be determined whether it is appropriate to continue the administration by evaluating the risk and the benefit reported by the patient.

Precautions for use

It is not administered to patients with pathologically serious heart disease (heart failure, serious valve disease, etc.).

It is not administered to patients with moderate and severe liver disorders.

It is not administered to patients receiving MAO inhibitors or to patients within 14 days after discontinuation of MAO inhibitors.

MAO inhibitors should not be administered within 7 days after discontinuation of polyfoxetine administration. It should not be used in combination with serotonin-like drugs and biopharmaceuticals, and is not administered to patients within 14 days after discontinuation of administration. These drugs should not be administered within 7 days after discontinuation of polyfoxetine administration.

Some potent hepatic metabolic enzyme inhibitors (ketoconazole, itraconazole, ritonavir, saquinavir, etc.) are not administered to patients who are being administered.

It can cause orthostatic hypotension (hypertension caused by postural changes, such as lying down or sitting), so do not administer it if you have a history of orthostatic reactions or have similar reactions in orthostatic tests (blood pressure, pulse, lying, standing up).

Patients with major psychiatric disorders such as depression, anxiety disorders, suicidal tendencies, and schizophrenia have no clinical experience and cannot be excluded from the possibility of worsening symptoms, so they are not administered.

It is not administered to women, children and adolescents under the age of 18 and to older adults over 65.
